Bespoke Book Box Registration

By signing up for the Bespoke Book Box you give the Swampscott Public Library permission to access your library account
in order to check out materials for your box. We will not access your account for any other purpose.

The Bespoke Book Box is a one-time service. We have a limited number of spots available per month, so once we are at capacity
you will be placed on our wait list and will be notified when your box is ready. If you would like another box, you will have to fill
out this form again and be placed at the end of the wait list.
